Customer Declaration Form

All new customers are required to answer questions according to to the Act relating to measures to combat money laundering and the financing of terrorism, etc. Further, you may need to provide a TIN-number if you are subject to US taxes. At last, you need a permanent address registered at the Norwegian National Registry (Folkeregisteret).

Please visit one of our branches and bring a valid passport (or other valid identification documentation), along with the customer declaration form.
